Smart Forms That Do the Work for You

Some forms act on the answers the moment a client submits them. Instead of reading responses and typing the results into the platform yourself, smart forms analyse, file, and set things up for you automatically.

What is a smart form?

A smart form is a form that does something with the answers as soon as a client submits it. A regular intake form stores responses on the client's record. A smart form goes further: it can analyse the nutrition of meals a client reports, update their profile, or configure their goals, all without you lifting a finger.

  • The Food and Symptom Diary works out the nutrition of each meal a client logs and can push those meals into their diary.
  • The Meal Plan Preferences form fills in likes, dislikes, and restrictions so the plan auto-generator matches them right away.
  • Body measurement forms log results straight to the client's measurement history.
  • Goal-setting forms configure a client's calorie and macro targets automatically.

From food diary to nutrition analysis, automatically

When a client fills in the Food and Symptom Diary, Foodzilla works out the nutrition of every meal they describe. You see calories and macros for each entry without doing any manual analysis, and you can push those meals into the client's diary so their tracking history stays complete.

Preferences that flow into meal planning

The Meal Plan Preferences smart form saves a client's likes, dislikes, and dietary restrictions directly to their profile. The next time you auto-generate a plan, it already respects what they told you. New clients can go from intake to a personalised meal plan in one sitting.
