Release and waiver of liability

I, the undersigned, acknowledge that I am voluntarily participating in a personal training programme and physical exercise activities. I understand that physical exercise carries inherent risks including, but not limited to, physical injury, muscle strains, sprains, fractures, and in extreme cases, serious injury or death.

I confirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in physical exercise, except as disclosed above. I agree to inform my trainer immediately of any discomfort, pain, or symptoms during exercise.

In consideration of being allowed to participate in training sessions, I hereby release and hold harmless my personal trainer, their business entity, employees, and agents from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, injury, or death that may be sustained during my participation in training sessions.

I acknowledge that this waiver is binding upon my heirs, executors, administrators, and assigns. I have read this waiver carefully and understand its terms. I am signing this waiver of my own free will.
