How to Create Branded Intake Forms in Foodzilla
Foodzilla’s Form Branding settings let you personalise the appearance and business information shown at the top of every form you send to clients.
This helps make your intake forms, questionnaires, consent forms, check ins, and other client forms feel consistent with your practice and gives clients clear information about who the form is coming from.
How to Access Form Branding
- Go to Forms & Bookings
- Open the Form Branding tab
- Add or update your branding details
- Click Save
Your form branding is applied across your public Foodzilla forms.
Form Branding Options
Profile Photo
Upload a square JPG or PNG image.
Your photo appears at the top of your public forms and helps clients immediately recognise who the form is from.
Role or Title
Add your professional title, such as:
• Accredited Practising Dietitian
• Nutritionist
• Health Coach
• Sports Dietitian
This helps establish your professional role when clients open the form.
Tagline
Add a short description of your practice or approach.
For example:
Helping busy people build healthier habits with practical, sustainable nutrition.
A clear tagline gives clients quick context about your services.
Qualifications
Add professional qualifications or certifications relevant to your practice.
For example:
• APD
• BSc Nutrition & Dietetics
• Sports Nutrition Certified
You can add multiple qualifications, which will appear alongside your branding information.
Website URL
Add your business or practice website so clients can easily learn more about your services.
Contact Email
Add the email address you want clients to use if they need to contact you about the form or your services.
Booking Link
Add your Foodzilla booking link or another appointment booking URL.
This makes it easy for clients to move from completing a form to booking a consultation.
Where Does Form Branding Appear?
Your branding details appear at the top of forms you send through Foodzilla.
Only information you add will be displayed, so you can keep your form branding as simple or detailed as you prefer.
